Outreach coordinators work closely with families to help them navigate the service eligibility process. The coordinators make sure people with disabilities are aware of services that apply to their individual circumstances, help them determine if they qualify, and then gain access.

In their role as case advocates, the coordinators also provide crisis intervention, counseling, problem resolutions, money management services, public advocacy, and drop-in supports as necessary to help individuals with disabilities achieve and maintain independent lives.
